C-support is a foundation that works on behalf of the Ministry of Health, Welfare and Sport (VWS). C-support supports and advises people with post-COVID and long-term complaints after vaccination. We also share our knowledge with professionals, researchers, policy makers and the government.
Who is responsible for the processing of personal data?
C-support, located at Goudsbloemvallei 1, 5237 MH 's-Hertogenbosch, is responsible for the processing of your personal data as mentioned in this privacy statement.
Personal data is data that says something about a person. For example, someone's name, address, telephone number and passport photo. But also information about where someone works and whether someone has post-COVID complaints or long-term complaints after vaccination.
Why do we process personal data?
C-support needs your personal data to be able to support you as best as possible with the complaints and the consequences of the condition. C-support has been instructed by the ministry to:
- To look together with the patient at the questions and needs that arise as a result of the disease;
- Connecting areas where collaboration is not always easy. For example, the medical world, agencies around work and income and the support available in participation and self-reliance by municipalities.
C-support also needs patient data for complaint handling. If a complaint needs to be handled, C-support needs to be able to communicate with the patient. For this, C-support needs patient data.
C-Support processes data for analysis purposes. Thanks to these analyses, we can optimize the support of the patient.
Finally, C-support also conducts scientific research. Personal data of patients are only processed if the patient has given permission for this. C-support informs patients clearly about the purpose of the research before the research. Only if patients agree to sharing data, the data is used for conducting research.
What data do we process?
Personal data that we process are:
- Name
- Address
- Date of Birth
- Gender
- (Mobile) phone number
- Email address
- Any test result COVID-19 test
- Possible COVID-19 vaccinations
- Contact details GP
- Referral from GP
In order to support you, a digital patient file is created. The advisor who supports you has access to the digital patient file. The advisor is part of a broader team within C-support. This team consists of other advisors who, in the context of, among other things, the quality and continuity of care, think along if necessary. This concerns, for example, a (substantive) manager of the team. It is therefore possible that the digital patient file is viewed by an advisor other than the advisor you are in contact with. There are free fields in the file. You can enter the following data yourself:
- Financial situation
- Social functioning/network/support
- Patient's request for help
- Mental well-being
- Physical health
- Limitations and diagnosis
- Medical records
- Working Conditions
If you enter this information, we will also store this information.
Can we process the data?
C-support processes personal data based on the basis of consent. When you register with C-support, we ask you for permission to use the personal data. Before you give permission, we indicate why and which personal data we collect and for what purpose. You may also withdraw your permission at any time.
In some cases, C-support processes personal data on the basis of legitimate interest. C-Support only relies on this basis to be able to use personal data for analysis purposes.
C-support asks for your permission to process data for scientific research. Without your permission, no data will be processed in the context of scientific research.
How long do we retain your personal data?
C-support stores the data in the aftercare process based on the basis of consent. If you withdraw that consent, or request the deletion of your data, we will delete the data. However, this does mean that C-support can no longer help you. You can always register again.
If you have given permission for the processing of data for scientific research, we will retain it for 15 years.
Is data shared with third parties?
C-support only provides data to third parties if this is necessary for the execution of our agreement with you or to comply with a legal obligation. We conclude a processing agreement with companies that process personal data to ensure the same level of security and confidentiality of the data. C-support remains responsible for these processes.
Cookies, or similar techniques, that we use
C-support only uses technical and functional cookies. If you give permission, C-support also uses analytical, performance and advertising cookies.
A cookie is a small text file that is stored on your computer, tablet or smartphone when you first visit this website. The cookies we use are necessary for the technical operation of the website and your ease of use. They ensure that the website works properly and remember, for example, your preferences. We can also use them to optimize our website. You can opt out of cookies by setting your internet browser so that it no longer stores cookies. In addition, you can also delete all information that was previously stored via the settings of your browser.
How can you view, change or delete your data?
You have the right to view, correct or delete your personal data that we record. In addition, you have the right to withdraw any permission for sharing the personal data. You can also object to the processing of the personal data by C-support. Finally, you have the right to data portability. This means that you can submit a request to us to send the personal data that we have about you in a computer file to you or a third party.
You can send a request for access, correction, deletion, data transfer, withdrawal of consent or an objection to the processing of your personal data to info@c-support.nu.
To ensure that such a request has been made by you, you must send a copy of your ID with the request. In connection with the protection of your privacy, you must black out the MRZ (Machine Readable Zone, the strip with numbers at the bottom of the passport), the passport number and Citizen Service Number (BSN) in this copy. We will respond to such a request as soon as possible, but certainly within four weeks.
How do we protect personal data?
C-support takes the protection of your personal data seriously and takes appropriate measures to prevent misuse, loss, unauthorized access, unwanted disclosure and unauthorized modification.

C-support has appointed a Data Protection Officer (FG). This officer monitors the use of personal data by C-support. For questions or comments about personal data in general and about this privacy statement, you can contact the FG via: dataprotection@c-support.nu.
To file a complaint
If you believe that we are not acting in accordance with privacy legislation, you have the right to file a complaint with the aforementioned FG (as an independent supervisor). In addition, you can also file a complaint with the Dutch Data Protection Authority.
